Tips for calling about active claim? Need help with forms sent by NYDOL

Hey everyone, I'm about to call about an active claim that needs some info to get paid. They sent me a bunch of forms and I have questions. Got all my docs ready to go. Anyone have tips on what to say to the agent?

Pro tip: I found a way to actually get someone on the phone without waiting forever. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
