Name spelling error on EDD SDI medical certification - can't reach anyone to fix it!
I just had my baby three weeks ago and now I'm dealing with this frustrating name error on my disability claim! When I submitted my claim through SDI Online, I entered all my information correctly. However, my OB/GYN's office apparently misspelled my last name on the medical certification form (DE 2501) they submitted by mail. \n\nNow I received my official notice from EDD and they've got my name wrong throughout the entire document. I'm worried this is going to cause problems with my benefit payments or tax documents later. \n\nI've been trying to call EDD for 5 days straight now, calling as soon as they open at 8am, but either get a message saying they're experiencing high call volume or I'm on hold for 45+ minutes before getting disconnected. \n\nDo I need to ask my doctor to submit a corrected form? Is there another department I should be contacting? Has anyone dealt with name errors on their disability claim before? Getting desperate here as bills are piling up during my maternity leave!

This happened to me last year with my pregnancy disability! Contact your doctor's office first and ask them to submit an amended medical certification with your correct name spelling. Then you need to send a message through your SDI Online account explaining the situation. Go to 'Contact SDI' and select 'Claim Information' as the category. Explain the error clearly and mention that your doctor will be submitting a corrected form. \n\nAlso, send a separate message through your account with the subject line 'URGENT: NAME CORRECTION NEEDED' and include both the incorrect and correct spelling of your name, plus your claim ID number. The name correction process can take 2-3 weeks but shouldn't stop your payments if you're otherwise eligible.

omg same thing happened to me!!! but they spelled my FIRST name wrong can u believe it??? its been 6 weeks and still not fixed ugh. make sure to keep calling them everyday dont just rely on the online messages

Yes! It really worked for me. My name issue was resolved in about 10 days total after I talked to the agent. They told me exactly which forms to have my doctor resubmit and added notes to my account about the correction being in process.

You actually get both! First you get Pregnancy Disability (which is part of SDI) for 4 weeks before birth and 6-8 weeks after birth depending on delivery type. Then after your doctor releases you from disability, you can transition to Paid Family Leave for bonding time. But they're processed through the same EDD system, which is why this name error is stressing me out so much.

To clarify: Pregnancy Disability Leave (PDL) provides up to 4 weeks of benefits before birth and 6 weeks after a vaginal birth or 8 weeks after a C-section. Then Paid Family Leave (PFL) provides up to 8 weeks of benefits for bonding. They're separate benefits but both administered through EDD, and you'll need medical certification for both. Start planning early because the paperwork can be confusing!
