EDD claims my doctor didn't send forms, doctor says they did - How to resolve this SDI claim issue?

So I'm having this issue with my unemployment claim. I called the office and they said my medical provider never sent in the forms. But here's the thing, I talked to my doctor's office and they swear they sent everything in. I'm stuck in the middle and don't know what to do. Has anyone else dealt with this kind of mess? It's so frustrating when you're trying to get your benefits and everyone's pointing fingers at each other. Any advice on how to sort this out?
