EDD SDI fax number for form submission - anyone know the right one?
Has anyone successfully faxed forms to EDD disability? My doctor's office says they prefer to fax my medical certification rather than using SDI Online, but they need the correct fax number. I searched the EDD website but found different fax numbers for different offices. My claim is for a back injury that's keeping me out of work for at least 8 weeks, and I don't want any delays because the form went to the wrong department. Any help appreciated!

The current fax number for EDD SDI medical certification forms (as of 2025) is (855) 341-6574. Make sure your doctor includes your full name, claim ID, and last 4 of your SSN on EVERY page they fax. Also have them keep the fax confirmation sheet - you wouldn't believe how many forms EDD claims they never received. I'd strongly recommend asking your doctor to submit through SDI Online if possible though - it's much faster and there's a clear digital trail.

•I want to add something important here - always call EDD about a week after your doctor faxes anything to confirm they received it! My doctor faxed my certification twice and somehow EDD lost both faxes. I ended up waiting 6 weeks with no benefits when my claim should have been approved right away. The fax system is really unreliable.

•That's actually an older fax number that some EDD materials still show. They consolidated their fax services in January 2025. The number I provided above should work for all SDI claims including pregnancy disability.

Here's a pro tip from someone who's been through this process multiple times: Have your doctor's office put YOUR NAME and CLAIM ID NUMBER in the fax cover sheet's subject line. This helps EDD route it to the right claim file immediately. Also, if they're going to fax, they should always call the EDD Medical Provider Helpline at (855) 342-3645 to verify the correct fax number for your specific claim type. The numbers sometimes change based on claim volume and processing center.

•For 2025, they're using the same fax number for both initial and continuing certifications. But for extensions specifically, make sure your doctor writes "EXTENSION REQUEST" clearly at the top of the form. This flags it for priority processing.

I work at a medical office and deal with EDD fax submissions regularly. Just want to confirm that (855) 341-6574 is correct - we've been using it since January 2025 with good success. One thing I'd add is to have your doctor's office send the fax during business hours (8am-4pm) if possible. We've noticed better delivery rates during peak staffing times. Also, if your doctor's office has a multi-function printer, make sure they're using "Fine" resolution mode - we had issues with EDD rejecting forms that were too blurry from standard resolution faxes.
