EDD Eligibility Questionnaire Never Received - Payments at Risk

Hey everyone, I'm in a bit of a pickle here. I was supposed to get this eligibility questionnaire to continue my benefits, but it never showed up in my inbox. Now my claim says I need to turn it in to keep getting payments. Has anyone else dealt with this? What should I do?

This happened to me last month! Here's what I did: 1. Check your spam folder. Sometimes EDD emails end up there. 2. Log into your EDD account online and look for any messages or alerts. 3. If you still can't find it, try to request a new questionnaire through your online account. 4. As a last resort, send a message through the EDD website explaining the situation. Hope this helps! Great advice! I'd add that taking screenshots of everything you do can be helpful if you need to prove you tried to resolve the issue.

I found a way to actually get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ
