Do doctors use same receipt number for extending SDI claims or need a new one?
I've been on disability for 3 months due to a back injury that required surgery. My doctor thinks I need 6 more weeks of recovery time before returning to work. When he submits the extension paperwork, should he use the same receipt number from my initial claim (starts with DE2501) or does he need to generate a new receipt number for the extension? The EDD website isn't clear about this and the last time I called, I waited for 2 hours and never got through. My benefits are scheduled to end next week so I'm getting anxious about this. Don't want to have any gap in benefits because my savings are practically gone at this point.

Your doctor needs to use the SAME receipt number from your original claim. The receipt number is tied to your entire disability period, including any extensions. Make sure your doctor completes and submits the 'Physician/Practitioner's Supplementary Certificate' (DE 2525XX) form before your current certification period ends. This form specifically requests the receipt number from your initial claim, so using the same one is correct and necessary.

•Thank you so much! This is exactly what I needed to know. I'll make sure my doctor has the right receipt number when filling out the extension form.

Had the same question last year and was so confused! Yes, its the same receipt # for the whole claim period, extensions included. Make sure ur doc submits the form at least 5-7 days b4 your current certification ends so theres no gap. My doc submitted late & I had 2 weeks with no pay while they processed the extension 😫

•Oh no, that's exactly what I'm afraid of! I'll definitely remind my doctor to submit it early. Did you have any trouble getting backpay for those two weeks once the extension was approved?

•They did backpay me eventually but it took like another 3 weeks to get it all sorted out. Such a pain. I had to call EDD like 50 times to get through.

i think its the same number... at least thats what my dr did for my pregnancy disability when i needed extra time after my c-section. used the original number and everything went fine.

Actually guys let me clarify something - there are TWO different numbers we're talking about here: 1. The RECEIPT number - this is what your doctor initially gets when they first submit your medical certification 2. The CLAIM number - this is what EDD assigns to your entire disability claim For extensions, your doctor needs to use your CLAIM number on the DE 2525XX form, NOT the receipt number. Many doctors confuse these! Make sure they use the claim number from your EDD account (usually starts with D01 or similar). If they use the wrong number, it can delay your extension approval by weeks.

•Wait, now I'm confused. So it's NOT the receipt number that starts with DE2501? It's a different number entirely?

•The previous commenter is partially correct but creating some confusion. For clarification: When your doctor initially files your medical certification, they get a RECEIPT number (from form DE 2501). However, once your actual claim is established, you get a CLAIM number from EDD. For the extension form (DE 2525XX), your doctor should use your CLAIM number, which you can find in your SDI Online account or on any correspondence from EDD regarding your claim. This number typically starts with D01 followed by numbers. Sorry for any confusion in my first response - I should have been more specific about which number to use.

had the WORST time with my extension last year!! made sure my Dr had all the right #s but EDD still messed it up and my payments stopped for over a month! kept calling the disability number and could never get through - busy signals ALL DAY LONG

•I had the same problem trying to reach EDD about my extension! I finally used Claimyr.com to get through to an agent in about 20 minutes instead of calling all day with no luck. They connected me directly to an EDD rep who fixed my extension issue on the spot. Check out their demo video: https://youtu.be/1X-mEsLtbmQ?si=1hcSq3KFtCr4oAmd - it saved me weeks of stress when my claim was about to expire.

MY DOCTOR SCREWED THIS UP TWICE FOR ME!!!! He used a new receipt number both times and my extensions took FOREVER to process. Make sure they use your CLAIM NUMBER (the D01 number) on the extension form. And check check CHECK that they filled everything out correctly. My doctor's office staff had no idea what they were doing with EDD forms.

•This happened to me too! Doctors offices are so confused about EDD paperwork. I finally had to physically sit with the office manager and show her exactly what to put where on the form.

To address your question properly: 1. For SDI claim extensions, your physician needs to use your CLAIM number (starts with D01) on the DE 2525XX form, not the receipt number. 2. The receipt number (DE2501) was only for the initial medical certification. 3. Timeline is critical: Your doctor should submit the extension 5-10 days before your current certification ends. 4. You can proactively help by providing your doctor with: - Your correct claim number from your SDI Online account - The exact date your current certification period ends - A completed DE 2525XX form with your information already filled in 5. After submission, verify through your SDI Online account that the extension was received. This approach has the highest chance of ensuring continuous benefits without interruption.

•This is incredibly helpful, thank you! I have an appointment with my doctor tomorrow and I'll bring all this information with me. I really appreciate the detailed explanation and checklist.

One more important thing - make sure your doctor includes detailed medical justification for WHY you need the extension. EDD is getting stricter about approving extensions in 2025, and they're requiring more specific medical reasons. Have your doctor be very clear about your recovery timeline and functional limitations. Just saying "patient needs more time" isn't enough anymore.

•That's good to know. My doctor has been documenting my recovery progress with physical therapy measurements, so hopefully that will help justify the extension. I still can't lift more than 10 pounds without pain, and my job requires lifting up to 50 pounds.

Sophia Gabriel
•That's perfect - having those specific functional measurements and comparing them to your job requirements is exactly what EDD looks for now. Make sure those specific details are included in the extension certification.

Just went through this exact situation 2 months ago! Your doctor definitely needs to use your CLAIM NUMBER (the D01 number) on the extension form, not the original receipt number. I made the mistake of giving my doctor the wrong number initially and it delayed my extension by 3 weeks. Pro tip: Log into your SDI Online account right now and screenshot your claim number and current certification end date. Bring both to your doctor appointment so there's no confusion. Also, since you mentioned your benefits end next week, you might want to call your doctor's office TODAY to make sure they can get the paperwork submitted with enough time. Some offices take a few days to process these forms internally before sending them to EDD. The good news is once they use the correct claim number and submit on time, extensions usually process pretty smoothly. Good luck with your recovery!

•This is such great advice, thank you! I'm definitely going to screenshot my claim info from my SDI Online account before my doctor appointment. I'm also calling his office first thing tomorrow morning to make sure they can prioritize getting the extension submitted ASAP. It's so stressful when you're relying on these benefits and worried about gaps in payment. Really appreciate you sharing your experience - it helps to know I'm not the only one who's dealt with this confusion!

I went through this same situation last year and want to echo what others have said - your doctor MUST use your CLAIM NUMBER (the D01 number) on the extension form, NOT the original receipt number. I learned this the hard way when my first extension got rejected because my doctor used the wrong number. Here's what I wish someone had told me: Call your doctor's office TODAY and ask to speak with whoever handles disability paperwork. Many offices have one specific person who knows how to do EDD forms correctly. Give them your claim number from your SDI Online account and your current certification end date. Ask them to confirm they can submit the DE 2525XX form by [specific date - at least 5 days before your benefits end]. Also, since you mentioned your savings are almost gone, you might want to look into other temporary assistance programs in your county while waiting for the extension to process, just as a backup plan. Some areas have emergency assistance funds for people between disability payments. Stay on top of this - don't assume your doctor's office knows what they're doing with EDD paperwork. Follow up the day after your appointment to make sure they actually submitted it. You've got this!

•This is incredibly helpful advice! I really appreciate you emphasizing the importance of following up - you're absolutely right that I shouldn't assume the doctor's office knows what they're doing. I've already had issues with them being slow with other paperwork in the past. I'm definitely going to call first thing tomorrow and ask specifically for whoever handles disability forms. The tip about looking into county emergency assistance programs is also really smart - I hadn't thought of that as a backup option. Thank you for taking the time to share such detailed guidance, it gives me a much clearer action plan to follow!

I'm going through this exact same situation right now with my shoulder surgery recovery! Based on what everyone's saying here, it sounds like the key is making sure your doctor uses your CLAIM NUMBER (the D01 number) from your SDI Online account, not the original receipt number. One thing I learned from my case worker is that you can also check the status of your extension online once it's submitted. Log into your SDI Online account and look for any updates under "Claim Status" - it usually shows when they receive the extension paperwork and when it's approved. This helped me avoid the anxiety of wondering if it went through. Also, if your doctor's office seems confused about the paperwork (like mine was), you can actually download a blank DE 2525XX form from the EDD website and pre-fill your personal information before bringing it to your appointment. This way you know all YOUR info is correct and they just need to fill in the medical parts. Hope your back surgery recovery goes smoothly and you get your extension processed without any gaps!

•Thank you so much for sharing your experience! The tip about pre-filling the DE 2525XX form is brilliant - I'm definitely going to do that before my appointment tomorrow. It'll save time and make sure there are no errors in my personal information. I also really appreciate the reminder about checking the claim status online - I had no idea you could track the extension paperwork that way. That'll definitely help with the anxiety of not knowing if it went through properly. Hope your shoulder surgery recovery is going well too!

I went through this same situation with my knee surgery last year and want to share what worked for me. Your doctor needs to use your CLAIM NUMBER (the D01 number from your SDI Online account) on the DE 2525XX extension form - NOT the original receipt number from DE2501. Here's my step-by-step process that prevented any payment gaps: 1. Screenshot your claim number and certification end date from SDI Online 2. Call your doctor's office and ask for the person who handles disability paperwork specifically 3. Bring the pre-filled DE 2525XX form to your appointment (you can download it from EDD website) 4. Make sure they submit it AT LEAST 5-7 days before your current benefits end 5. Follow up within 24 hours to confirm they actually sent it The most important thing is being proactive and not assuming your doctor's office knows the process. I had to educate my doctor's staff about which number to use because they were going to use the wrong one initially. Since your benefits end next week, I'd recommend calling your doctor's office first thing Monday morning to get this prioritized. Don't wait until your appointment - get the ball rolling now so there's time to fix any issues that come up. Good luck with your recovery - back surgeries need proper healing time, so don't rush it!

•This is such a comprehensive guide - thank you Omar! I really appreciate the step-by-step breakdown. You're absolutely right about being proactive rather than waiting. I'm actually going to call my doctor's office right now instead of waiting until tomorrow morning. The idea of pre-filling the DE 2525XX form is genius too - I'm downloading it from the EDD website as soon as I finish reading through all these responses. It's reassuring to hear from someone who successfully navigated this process without any payment gaps. I was starting to panic about running out of savings, but having this clear action plan gives me confidence that I can get this handled properly. Thanks again for taking the time to share your experience!

As someone who just went through this process for a work-related injury, I can confirm what others have said - your doctor needs to use your CLAIM NUMBER (the D01 number) for the extension, not the original receipt number. One thing I haven't seen mentioned yet is that you should also verify your doctor has your current work restrictions documented correctly on the extension form. Since you mentioned your job requires lifting 50+ pounds but you can only lift 10 pounds, make sure those specific functional limitations are clearly spelled out in the medical justification section. EDD has been more thorough about reviewing these details lately. Also, if you're worried about a gap in benefits, you might want to set up text alerts in your SDI Online account so you get notified immediately when your extension is processed. That way you'll know right away if there are any issues that need to be addressed. The fact that you're being proactive about this now (even though you're stressed) puts you in a much better position than people who wait until after their benefits stop. You've got this!

•Thank you Sophia! That's such an important point about documenting the specific work restrictions. I hadn't thought about setting up text alerts either - that's really smart. I'm definitely going to enable those notifications so I know immediately when the extension gets processed. It's helpful to hear that being proactive now puts me in a better position. I was feeling overwhelmed but all these detailed responses are giving me confidence that I can handle this properly. Really appreciate you mentioning the functional limitations piece too - I'll make sure my doctor is very specific about the 10 pound vs 50 pound lifting restriction when filling out the form.

I'm new to this community but going through a similar situation right now with my wrist surgery recovery. Reading through all these responses has been incredibly helpful! Just want to add one more tip that my case worker mentioned - if you're really worried about timing, you can also fax the DE 2525XX form directly to EDD in addition to having your doctor submit it through their normal process. The fax number is on the form itself. This gives you a backup in case there are any delays with your doctor's submission method. Also, keep a copy of everything for your records - the completed form, fax confirmation if you use that option, and screenshots of your SDI Online account showing your claim number and current certification dates. Having documentation has saved me multiple times when dealing with EDD. Hope your back surgery recovery goes smoothly and you get your extension approved without any payment gaps!

•Welcome to the community Ruby, and thanks for that great tip about faxing directly to EDD as a backup! I hadn't thought of that option but it makes total sense to have a safety net in case there are delays with the doctor's office submission. The documentation advice is also really valuable - I've learned from other EDD experiences that having everything in writing can save you so much hassle later. It's reassuring to connect with others going through similar situations. Hope your wrist surgery recovery is going well too! This community has been such a lifesaver for navigating all the confusing EDD processes.

I'm new here but dealing with the exact same situation after my ankle surgery! Reading through all these responses has been SO helpful - I was completely confused about which number to use and my doctor's office had no clue either. Based on everything I'm seeing here, the key points seem to be: 1. Use your CLAIM NUMBER (D01 number) from SDI Online, NOT the receipt number 2. Submit the DE 2525XX form at least 5-7 days before benefits end 3. Pre-fill your personal info on the form before the appointment 4. Follow up within 24 hours to confirm submission 5. Set up text alerts in SDI Online to track the extension Gabriel, since your benefits end next week, definitely call your doctor's office TODAY if you haven't already. Don't wait for your appointment tomorrow - get them started on preparing the paperwork now so there's time to fix any mistakes. Also want to echo what others said about being very specific with functional limitations. My job requires standing for 8+ hours but I can only stand for 20 minutes at a time right now, so I made sure my doctor included those exact details on my extension form. This community is amazing for sharing real experiences with the EDD process. Thanks everyone for the detailed advice!

•Welcome Lia! Your summary of the key points is spot-on and really helpful for anyone new to this process. I'm also dealing with a post-surgery recovery (herniated disc repair) and was initially so confused about all the different numbers and forms. This community has been a lifesaver! Your point about being specific with functional limitations is crucial - I learned that the hard way when my first extension request was too vague. Now I make sure my doctor documents exactly what I can and can't do in measurable terms (lifting limits, walking distances, sitting/standing time, etc.) and compares it directly to my job requirements. For anyone reading this thread, another tip I'd add is to ask your doctor's office for a copy of the completed form before they submit it. This way you can double-check that they used the right claim number and included all the important details. I caught an error this way that would have definitely delayed my extension. Gabriel, hoping your extension goes smoothly! The fact that you're being proactive about this gives me confidence you'll get it sorted without any payment gaps.
