New York Unemployment saying I lied on application - Was fired from McDonalds over slushie incident

Got fired from my fast food job as shift manager after trying to mix flavors when we ran out of a slushie flavor (never served it). Put 'lack of work' on unemployment app but employer reported 'policy violation'. Now unemployment wants me to pay everything back. Should I request a hearing?
