Never received State Tax refund but always get Federal - what's going on?

I file my taxes every year but I only ever get my federal refund. Never seen my state refund hit my account. Been happening for years now and im starting to think something's wrong. Is there a reason why this keeps happening? Anyone else dealing with this?

Alaska, Florida, Nevada, South Dakota, Tennessee, Texas, Washington, and Wyoming don't have state income tax. New Hampshire only taxes investment income.
