Switching from maternity to baby bonding PFL - who provides the application forms?

I'm so confused right now! My pregnancy disability leave (maternity) is ending in 2 weeks, and I want to apply for the 8 weeks of PFL baby bonding time I'm entitled to. But nobody seems to know who's supposed to give me the application forms! My OB's office told me today that my employer should provide them. When I asked my boss, he just shrugged and said he'd "have to get back to me" because he "doesn't know what that would be." Meanwhile, my leave clock is ticking! I'm starting to panic a little because I don't want to miss the deadline and lose my bonding time. Is this something I need to get from EDD directly? Do I download forms online? Does HR handle this? My company is pretty small (only 28 employees) so our HR department is basically just my boss and the payroll lady who works remotely. When I had my baby last year they gave me all the forms at the hospital before I went home? Maybe check the package they sent you home with, it might be in there somewhere. Or maybe call the hospital and see if they have it?
