Where to find Reduced Hour Wages form? Can't locate it anywhere!

Hey everyone, I need some help. I'm trying to get my hands on a reduced hour wages form but I can't seem to find it anywhere. Has anyone else had this issue recently? Where should I be looking for this form? Any advice would be really appreciated!

Actually, I found a way to get someone on the phone without the wait. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
