SDI deposited $488 but it's missing, surgeon can't submit docs online, might have to cancel surgery due to low payments. Help!
I got a notice from SDI saying they deposited $488, but it never showed up in my account. They're only paying me under $1k a month, which isn't enough to live on, so I might have to cancel my surgery. My ortho surgeon can only mail docs to SDI cause he lost his password. I'm not sure if I should mail the letter or if the doctor needs to do it. Anyone know if there are specific forms we need to fill out for this?

I found a way to actually get someone on the phone! I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ

Regarding the forms, there should be a specific form for your doctor to fill out. It's called the DE 2501 I think. Your doctor needs to mail it in, not you. Make sure they include all the necessary medical info!

OK, let me break this down for you: 1. Missing deposit: Call SDI ASAP. Sometimes there are glitches in their system. Keep all your notices as proof. 2. Low payments: Unfortunately, SDI payments are based on your past wages. Not much you can do to increase it. 3. Doctor's documentation: Your doctor needs to fill out form DE 2501. They MUST mail it. You can't do it for them. 4. Cancelling surgery: Talk to your doctor first. There might be other options or payment plans available. 5. Doctor's password issues: Tell them to call SDI's technical support line. They have a separate number for medical offices. Hope this helps!
