Need help with SDI medical forms - can't get through on phone. Any advice?

Hey everyone, I'm trying to get more info about some medical forms for SDI. Been on hold forever and can't seem to get through to anyone. Anyone else dealing with this? Any tips on what I should be asking when I finally get someone on the line?

When you finally get through, make sure to ask about the specific sections of the form you're unsure about. Also, get the name of the person you talk to and ask for a reference number for the call. It helps if you need to follow up later.

I actually found a way to get someone on the phone without waiting forever. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's what you need to know about SDI medical forms: 1. Make sure you have the correct form. For most cases, it's the DE 2501 Claim for Disability Insurance Benefits. 2. The form has several parts. You fill out Part A, your doctor completes Part B, and your employer does Part C. 3. Be super careful about the dates. They need to match up with what your doctor puts down. 4. If you're confused about any section, leave it blank and ask the rep when you get through. Better to leave it blank than put wrong info. 5. Keep copies of EVERYTHING. I can't stress this enough. 6.
