UCC 3 addendum form confusion - additional collateral schedules
Getting really frustrated with our state's UCC-3 amendment process. We need to add equipment collateral to an existing UCC-1 that only covered accounts receivable originally. The SOS website mentions a 'UCC 3 addendum form' for additional collateral schedules but I can't locate the actual addendum document anywhere in their forms library. Called the filing office twice and got different answers each time - one rep said use the standard UCC-3 amendment with attachment, another said there's a specific addendum form required for collateral additions. Has anyone successfully filed a UCC-3 with additional collateral using an addendum form? Really need to get this amendment filed before our loan closes next week.

I think you might be overcomplicating this. Most states just use the standard UCC-3 amendment form and you attach additional pages for expanded collateral descriptions. Never heard of a separate 'addendum form' specifically.
