UCC 3 form PDF - where to find the correct version for amendments?
Having trouble locating the right UCC 3 form PDF for filing an amendment. Our lending department needs to update collateral descriptions on several existing UCC-1 filings but I'm seeing different versions of the form online. Some look like they're from 2019, others don't have the right checkbox options for amendments vs terminations. Anyone know where to get the current standardized UCC-3 form that most states accept? We've had two filings rejected already because apparently we used an outdated version. Really need to get these amendments processed this week before our compliance audit.

Most Secretary of State offices have their own UCC-3 PDF on their website. The form varies slightly by state even though it's supposed to be standardized. What state are you filing in?

Been there! The trick is to always download the form directly from the state's SOS website right before you file. Don't use cached versions or forms from other sites. I learned this the hard way after getting three rejections in a row.

UCC-3 amendments are tricky because you have to get the debtor name EXACTLY as it appears on the original filing. One wrong middle initial and you're toast. Are you sure the rejections are just about the form version?

Pro tip: if you're doing amendments frequently, bookmark the direct links to each state's current UCC-3 form. Michigan updates theirs every January, Ohio every 6 months, Illinois whenever they feel like it apparently.

Are you filling out the amendment section correctly? Common mistake is checking 'termination' instead of 'amendment' or not specifying what exactly you're amending in the collateral description box.

Side note - anyone else notice that the PDF forms from some states don't fill out properly in Adobe? I've had better luck with Chrome's built-in PDF viewer for filling out UCC-3 forms.

Update: Found the current forms for all three states and got the amendments filed successfully. Turns out two of the rejections were actually due to missing filing fees (duh) and one was the old form issue. Thanks everyone for the help! That Certana tool someone mentioned actually caught a small discrepancy in one of the debtor names that I would have missed.
