Need advice: Getting medical provider to fill out disability claim forms before deadline

I need to get my medical provider to fill out some forms for my disability claim. The office closes at 5 and it's already late on a Friday. I'm worried about getting this done in time. Any advice on how to handle this or what to expect?

I found a method to get someone on the phone quickly. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works.
