I received a "Notice of Return to Certification Payment", but never received a form to return to EDD. have been stopped for a month, and I do not know why. Does anyone know which form is needed?

Have you had more than 10 weeks of payment? They only send Disability Claim Continued Eligibility Questionnaire (DE 2593) after 10 weeks of payment.

•Yes. My claim has been paying longer than 10 weeks. I have submitted DE 2500A seven times already. There is no new 25000A that is available to submit. The doctor's documentation is also up to date.
