Alabama UCC statement request form - where to find official version?

I'm helping a client verify some collateral information and need to request copies of existing UCC filings in Alabama. Does anyone know where I can find the official Alabama UCC statement request form? I've been looking through the Secretary of State website but getting lost in all the different forms. We need to pull records for a equipment financing deal and want to make sure we're using the right paperwork. The debtor has some pretty complex business names so I want to double-check everything is filed correctly before we proceed with our own UCC-1. Any guidance on the proper Alabama UCC statement request form would be really helpful!
