IRS keeps rejecting return - missing refund prequalification form error

Third time trying to file my taxes and keep getting rejected because of this refund prequalification form. Never had to fill this out before. Can't find it anywhere on the IRS website or in my tax software. Really frustrated and running out of time. Anyone know what this is or how to fix it?

Tax pro here. There's actually no such thing as a 'refund prequalification form' - this sounds like an error code issue with your tax software. Which software are you using? Most likely its picking up an inconsistency in your data entry that's triggering this generic error message.
