They're letting me do the 5 monthly payments of $100 each, just like I proposed in my offer. It's such a relief compared to what I was paying before. As for conditions going forward, yes there are a few important ones. I have to stay completely current on all tax filings and required payments for the next 5 years. If I miss any tax returns or owe additional taxes during that period, the IRS can revoke the entire agreement and I'd be back on the hook for the original amount (minus whatever I've paid). Also, any tax refunds I would've received for the tax year when the offer was accepted get kept by the IRS, but that's a small price to pay.

When I did an OIC a few years ago, my credit actually improved because the tax lien was removed after acceptance. The IRS files a Notice of Federal Tax Lien when your tax debt gets high enough, and that definitely hurts your score, but once your OIC is accepted and paid, they remove it.

The documentation was definitely the most time-consuming part! Here's what I had to submit: I filled out Form 656 (the actual Offer in Compromise) and Form 433-A (Collection Information Statement). For the 433-A, I had to provide three months of bank statements for all accounts, pay stubs, proof of all my expenses (rent receipts, utility bills, car payment, insurance, medical expenses, etc.), and verification of any assets I owned. The key was being super detailed about my expenses and making sure I had documentation for everything.
