Was a bit nervous or untrusting at first, but my calls went thru. First time the wait was a bit long but their customer chat line on their page was helpful and put me at ease that I would receive my call. IRS Rejected My Return - 1095-A From Florida Health Care Plans in My Account While I've Only Ever Lived in Ohio - Identity Theft Concern

My 2023 return just got rejected because of a missing 1095-A form. After checking my IRS account today, I found a Form 1095-A Health Insurance Marketplace Statement from FLORIDA HEALTH CARE PLANS used with my information. I've never lived in Florida - been in Ohio my whole life with W2s to prove it. When I logged into my IRS account and looked under "Showing all Forms | Viewing Most Recent to Oldest Date," there it was, clear as day - a 1095-A from Florida Health Care Plans. I don't even have a spouse who could have signed into their own account to view this - it's just me, and I've only ever had Ohio insurance. The IRS website specifically notes "If you're married: Your spouse must sign in to their own account to view information returns issued in their name." But I'm not married, so there's no possibility of confusion there. In the 2024 section of my account, under "Showing all Forms | Viewing Most Recent to Oldest Date," I can clearly see: Form and Title: 1095 A Health Insurance Marketplace Statement Payer Name: FLORIDA HEALTH CARE PLANS The IRS site also mentions that this section doesn't "include state or local tax information that is reported on the original form." But that doesn't explain why I'm seeing Florida health insurance documentation when I've never set foot in Florida. How complicated is this gonna get? Has anyone dealt with something similar? This feels like identity theft. Should I be contacting both the IRS and Florida Health Care Plans about this? Will this delay my return even further? I've only ever had Ohio insurance through my employer.
