If your submitted the DE 2525XX form, you should be able to and your old one if it's within 52 weeks of the original disability claim window. That would be easiest. A new claim may be needed if it's been over a since you first started claiming benefits.

This is usually easier and faster than starting a new claim. You'll need your to complete the DE 2525XX form (Physician/Practitioner's Supplementary Certificate) to your disability period. If it's been over a since your original claim started, then you'd need to file a new claim.
