Help! Urgent form submission needed to keep benefits - what if I mess up?

Hey everyone, I'm freaking out a bit here. I need to submit this form to keep my benefits going, but I'm not sure about some of the details. Anyone else been through this? What happens if I mess up or miss the deadline? Really don't want to lose my benefits over some paperwork!
