Benefits stopped because return to work date had been reached. Just 2 days earlier disability deemed permanent. To continue had to file form 2525xx. Dr. filed right away and was received (was able to read it from docs submitted by physicians) Three days in, I received message to submit 2525xx. My benefits stopped and I hope this gets resolved soon. Anyone have any experience or advice? Appreciate it
