Never received my 1065 form or Schedule K-1 for LLC - what should I do now?

I started my first LLC business last year and I'm just now realizing business taxes were due March 15th instead of April 15th like regular taxes (ugh!). I never received any 1065 form or Schedule K-1 in the mail for my LLC. Nothing at all tax-related came for my business. I also just found out that I was supposed to pay for a local business tax receipt with my city before operating? Could that be why I never got any tax forms? I'm completely lost here.
