NYS Department of Labor payroll tax form - which one shows unemployment tax due?

I'm doing quarterly payroll taxes for my small business and I'm confused about which form actually shows the unemployment tax I owe to NYS Department of Labor. My accountant moved and I'm trying to figure this out myself. There are so many different forms and I want to make sure I'm looking at the right one that summarizes what I owe for unemployment insurance. Can someone help me understand which specific form shows the total amount due?

This form summarizes all your unemployment insurance tax liability for the quarter. yeah the nys-45 is what you need, i file mine every quarter and it has everything on there including the UI tax

Just remember the NYS-45 is due by the last day of the month following the end of each quarter. So Q4 2024 was due January 31st, 2025. Don't be late or you'll get hit with penalties!
