Just received a 1095-C form - Should I have included it in my tax return?

I just received a 1095-C form in the mail today, and I'm kind of worried because I already filed my taxes about two weeks ago. Was I supposed to include this information in my return? I'm wondering if it might possibly make a difference in my taxable income or maybe even adjust my refund amount? If ur still worried abt this, might be worth calling the IRS directly to confirm. I had a similar q last yr and spent literally 3 days trying to get thru to someone.
