EDD 2525 form never received - Need email & extension. Help!

Hey everyone, I'm in a bit of a pickle. I never got the 2525 form in the mail for supplemental certification. I need it sent to my personal email and the due date extended. Has anyone else dealt with this? I'm worried about getting charged extra fees if it takes a long time to sort out. Any advice?

I found a method to get someone on the phone. I used this site's (claimyr.com) calling tool that dials the phone menu, stays on hold, waits for a live agent to pick up, then sends the call to my phone. It does cost $20, but it works. Here's a video about it: https://youtu.be/1X-mEsLtbmQ

They don't guarantee it, but they keep trying until they get through. It took about 2 hours for me, but I didn't have to sit by the phone the whole time. When they got an agent, my phone rang and I was connected.

Have you checked your spam folder? Sometimes EDD emails end up there. Also, make sure they have your correct email on file.
